January 15th Elementary Extravaganza Registration Form

Monday, January 15th; 8:30am-5:30pm | We are excited for an elementary extravaganza day this winter! Our day will include serving at Becker County's Food Pantry at their new location, hyper bowling at KingPinz and bouncin' on all the things at the Frostival's Bouncin' Bash at the Rustad Rec Center! Cost includes any cost for activities, lunch and snacks. $25 due for this program. Talk to Laura with any questions. We hope you can join us!

Parental Permission
By entering my name in the box below, I am providing my digital signature on this form and give my child(ren) permission to participate in the elementary extravaganza and leave Trinity's grounds. I give permission to the Trinity staff to seek necessary medical treatment in case of illness or injury. I also give Trinity Lutheran Church permission to use any photograph/video of my child taken during this program in the future promotion of Trinity.
 
 
Payment
