By signing this form, I give my child permission to attend VBS, participate in all associated events both on-site and off, and authorize the Luther Crest staff & Trinity staff to seek necessary medical treatment in case of injury or illness. I understand Luther Crest assumes secondary insurance coverage; the child’s family assumes primary coverage. I also give Trinity Lutheran Church and Luther Crest permission to use any photograph/video of my child taken at VBS in the future promotion of Trinity or Luther Crest.
